2012 GNT Registration Form

This form will allow you to enter your team's pertinant information and send an e-mail to the GNT Distict Co-ordinator, Jeff Miller, informing him of your intent to enter the Grand National Teams for 2012.

It does reasonable checking BUT will send you a copy of the e-mail generated to the e-mail that you supply in the form. Therefore, it is important for you to verify your contact e-mail and contact phone number prior to submission.

The CCBA will contact you (via e-mail) upon verifying the information, and entering your team into this year's event.

There will be a finalized form for you to print as your reference.

Please be sure that you are familiar with the Conditions of Contest for this year's event.
